it's either proxies or something in your config / code.  If you think the proxies are good, then I'd dump the html response out for a working account and compare it against a non-working to see if it might just be a parsing issue.  If it's not a parsing issue then you're likely submitting a payload incorrectly...something common is that you're not encoding the data, or encoding when you're not supposed to / double encoding.
