How to use? 

Get an ipvanish account (find one in my threads or elsewhere)

Login to your ipvanish account and navigate to

Open the txt file with your chosen editor, find and replace
find > insertuser > replace with > the user you took note of before > replace all
find > insertpass > replace with > the pass you took note of before > replace all

                                                                                          

Now enjoy your medium quality proxies
