We don't know the certificate you're looking for. It'd be great if you're able to provide us the right certificate, so we can work on it accordingly.
We can pick a random certificate, i.e. https://i.imgur.com/bHwFy3g.png which is a Nigerian certificate, but how do we know it's going to work for your specific case?

Hy I cn photoshop this for you easily. Only issue is you will need to tell me a doctors name that physically exists. As they will check the hospital address and doctors name and signature.
